Use the survey linked on the city’s Community Health Assessment page, or request a paper version at Falls Community Health. The city says the survey takes about ten minutes, offers translations and does not collect personally identifying medical information. This is a planning exercise, not an application for treatment, insurance or financial assistance.
The assessment combines residents’ experiences with focus groups and health data. Responses can identify obstacles such as difficulty finding an appointment or paying for care that may not be evident in records of completed medical visits. The city says the resulting assessment helps guide future health programs, services and investment decisions.
Current 2025–2028 priorities remain healthy living, adolescent mental health and access to care, including oral health. The survey does not immediately replace those priorities or commit funding to a particular service. The published November 30 date has no stated cutoff hour or timezone; check the city page for any revised participation instructions. For an accommodation, that page directs residents to the Human Relations Office at 605-367-8745 or human.relations@siouxfalls.gov by November 25.
