Applicant eligibility is narrower than simply being a nonprofit. The organization must be a certified Illinois SHIP site, active in the STARS reporting system, and able to administer subgrants across its Planning and Service Area. The notice covers the state’s 13 areas.
The application must identify proposed local recipients and funding amounts. Subgrants above $2,500 need an explanation in both the narrative and budget; approval depends on funding and individual review. The eligible-site list uses at least 25 client contacts during April 1, 2025–March 31, 2026, with provisions for qualifying new sites and approved exceptions.
Submit through the AmpliFund opportunity linked from the Department’s public notice before the cutoff. Late applications will not be evaluated. Portal questions go to Aging.gata@illinois.gov. Awards remain contingent on funding, and recipients must meet the notice’s financial and program-reporting requirements.
Residents should not use the grant application to request benefits. SHIP separately offers free Medicare counseling for beneficiaries and caregivers; its program page lists 1-800-252-8966 and Aging.SHIP@illinois.gov.
